- If you have a Medicare Advantage Plan, you will need an Authorization Letter from you Insurance company to be seen. Please call your Primary Care Physician's office for assistance.

- If you have an HMO plan, you will need an Authorization Letter from your Insurance company to be seen.  Please call your Primary Care Physician's office for assistance.

- If you have Medi-Cal, you will need an Authorization Letter from your Insurance company to be seen.  Please call your Primary Care Physician's office for assistance.

- If you have Kaiser Insurance, you must go to a Kaiser facility.  We cannot take Kaiser Insurance.
